How to Apply
The Application Process
A comprehensive and detailed application packet is vital for us to determine if The Achieve Academy is right for your child.
A complete application packet must include the following:

- Application form (Click here to complete the form)
- Non-refundable application fee of $100
- Most recent evaluations:
- Psychological, educational, or neuropsychological evaluation (if applicable)
- Speech/language evaluation (if applicable)
- Occupational therapy evaluation (if applicable)
- Current Individual education plan (IEP)
- Recent standardized test scores (i.e., FSA, SAT10, etc.)
- Current and Past Report cards
*The application fee is non-refundable.
